Structured forms are one of the most time-consuming and repetitive parts of clinical documentation. Whether it’s medical disability paperwork, insurance forms, or government documentation, clinicians often find themselves re-entering the same information across multiple sections.

Tali already helps streamline this process. After an encounter, you can generate structured documentation from your patient conversation. Alongside your SOAP note, Tali uses the transcript to populate one of our available fillable forms, helping reduce manual entry and save time.

But we know something important. Not everything needed for a form comes up during the appointment itself. Sometimes additional context is required. Sometimes details need clarification. Sometimes you need to reference past documentation.

How to Generate a Form in Tali

Using Form Helper starts with generating a form from an encounter.

  1. Create a new encounter
  2. Complete your patient conversation
  3. Click “Generate”
  4. Scroll to the bottom of the list to access “Forms”
  5. Select the form you would like to generate

If your form is missing information, you can either manually edit fields or use Form Helper to dictate additional context and upload supporting files.

Once complete, you can download or print the form directly from Tali.
